What diving medical services such as DAN do, how underwater cave rescue is organised in the USA, UK and France, why the nearest hospital may not treat a diving injury, why most cave incidents end in recovery, and why directory listings are not emergency routing.

When something goes wrong in a flooded cave, the help that exists is real but limited, and much of it arrives later than people imagine. This article explains what the main diving medical and cave rescue organisations do, why the nearest hospital is not necessarily able to treat a diving injury, and why a directory listing, including any on scuba.guide, is no substitute for official emergency routing. It contains no first aid or rescue procedures. In any emergency the first call is to the local emergency number.

Diving medical assistance

Divers Alert Network began in 1980 at Duke University Medical Center with the task of running a 24-hour hotline for injured divers [2]. Its emergency hotline page lists +1-919-684-9111 and says DAN medics are on call around the clock. The same page tells callers facing a life-threatening emergency to contact local emergency medical services, or take the injured person to the nearest medical facility, first, and only then to call DAN [1]. That number is published on dan.org by DAN, which is based in the USA; the page does not define a service region and describes the services as a membership entitlement (checked 2026-10-14). DAN is one of five independent regional organisations that make up the International Divers Alert Network [4], and its DAN World operation lists Asia-Pacific, Latin America and the Caribbean, and Canada among its service regions [17].

In Europe, DAN Europe's emergency page gives +39 06 4211 5685 as its international emergency contact number for members, offers round-the-clock assistance with diving medical officers in more than 20 languages, and likewise advises calling local emergency services or going to the nearest hospital first in a dangerous or life-threatening situation (daneurope.org, checked 2026-10-14) [5].

These services are medical. They put callers in touch with doctors who understand diving injuries, advise on the most suitable treatment facility, help arrange evacuation and, for members with cover, handle payment arrangements [1][5]. They are not rescue services, and they do not send divers into caves.

Why the nearest hospital, or chamber, may not be the answer

Some diving injuries, such as decompression illness, may need treatment in a hyperbaric (recompression) chamber. DAN is clear that the first step is assessment at a hospital or emergency clinic, not a drive to a chamber. Explaining why it does not publish chamber availability, DAN notes that divers have driven past medical facilities to reach a chamber; that doctors need to rule out conditions that mimic decompression illness, such as a heart attack; and that most chambers keep daytime hours, some are not staffed at night or at weekends, some do not treat divers at all, and their status changes with maintenance, staffing and patient load [3]. DAN Europe made the same points in 2015, adding that only a small and shrinking share of hyperbaric centres are trained and willing to treat diving injuries, and that many need advance notice from a doctor or DAN before they can take a diver [6].

The reverse also holds. A nearby general hospital is the right place for stabilisation, but it may have no chamber and no staff experienced in diving medicine. Helping doctors bridge that gap is what diving medical services are for [3][6].

Underwater cave rescue and recovery

Specialist cave diving rescue is organised differently from country to country, and it generally works through public authorities. In the United States, IUCRR, an all-volunteer organisation established in 1999, supports public safety agencies with rescue and recovery in caves, caverns and flooded mines, operating within the law enforcement agency's incident command system [7]. Its emergency page asks people reporting a diver overdue or missing in a cave, cavern or mine to email the organisation and contact the relevant Regional Coordinator [8]. A 2017 paper co-written by an IUCRR member describes its recovery procedure and strongly recommends that local dive teams do not attempt recoveries from flooded caves [9].

In the United Kingdom, the British Cave Rescue Council explains that responsibility for inland rescue rests generally with the police, who call on volunteer underground rescue teams, some of whose members can dive [11]. Its guidance for an emergency in the UK is to dial 999, ask for the police and then ask for cave rescue (caverescue.org.uk, checked 2026-10-14) [10]. In France, the Spéléo-secours français, a commission of the French Speleological Federation approved by the civil protection authorities for rescue in natural or artificial cavities, flooded or not, is listed with a free number for underground accidents, 0800 121 123 (ffspeleo.fr, checked 2026-10-14) [13]. At European level, ECRA exists to share knowledge and experience between cave rescue organisations; it does not present itself as an emergency dispatcher [14].

The hard limits

It is important to be candid about what rescue can achieve underwater. The published record is dominated by recovery rather than rescue. The 2017 paper on IUCRR's work is about recovering divers who have died, an average of three a year inside flooded caves in the United States [9]. In the Devil's Eye case in 2021 and the Cow Springs case in 2023, divers who had been diving alone were not reported missing until long after they entered the water, and IUCRR divers found them where they had died [18][19]. At Phantom Lake Spring in 2023, support divers could search only as deep as the gas they carried allowed, and it took the project team several days to bring their colleague out [15]. At Roaring River in 2022, teammates already in the water had to abandon attempts to help once visibility and the guideline were lost [16].

The 2018 Tham Luang rescue in Thailand is the exception that shows how much a successful cave diving rescue demands. It took an international team of experienced cave divers, extraordinary medical measures and a three-day extraction after the trapped group had been in the cave for more than two weeks. The British Cave Rescue Council called it extraordinary and unprecedented, and the doctors who described it cautioned against generalising its success to other diving scenarios [12][20].

Directory listings are not emergency routing

scuba.guide lists caves, dive sites, providers and organisations, and some pages mention nearby towns or facilities. None of that is emergency routing. A listing cannot tell you whether a hospital has a chamber, whether a chamber is staffed tonight, or which agency handles cave incidents in a given place, and contact details go out of date: a 2015 DAN Europe article gives an emergency number that differs from the one on DAN Europe's emergency page in October 2026 [5][6]. Who to call, the local emergency number and the way a diver's insurer or medical service wants to be contacted are things to confirm from official sources, for the specific place, before travelling. Every phone number in this article carries its jurisdiction, its source and the date it was checked, and each should be confirmed at that source before it is relied on.
